Whole Health Coaching Program Participant Agreement

This form is intended to provide you with information to help decide whether working with a Whole Health Coach is for you.

The Whole Health Coach Program aims to improve VEBA member health and wellbeing with the goal of providing compassionate support to members tailored to their specific health needs and desired goals for optimal wellness. The Whole Health Coaching program is designed with intention to be member-led, strengths-based, trauma-informed, and guided by best-practice. Discover your personalized path to wellness.

What is Whole Health Coaching?


A Whole Health Coach is someone trained to help you identify what matters most to you and work with you to set and achieve balanced goals based on your strengths, values, and identity. Whole Health Coaches empower VEBA members to work towards building the kind of life they wish to live.

Your Whole Health Coach will help you choose and work on goals that you find important for your life and health. For example, you may elect to focus on fitness-related goals and outcomes, while others may be more interested in improving their ability to perform better in their career, reduce stress or focus on their sense of purpose in life. What is "Whole Health"?


All goal setting achieved with a Whole Health Coach is toward achieving optimal wellness via a greater understanding of The Dimensions of Wellness and the significance of their interconnectedness.

What can I expect from my coach?


Your VEBA Whole Health Coach will help you realize your wellness vision and can support you in setting and achieving goals. Your coach is committed to ensuring that the time you invest in coaching is valuable and beneficial to you. Coaches may challenge you to think about what is truly important to you and help you to hold yourself accountable to achieving your goals but will always treat you with respect and value you as the expert in your own life. Your Whole Health Coach values your honesty and participation throughout the program for you to achieve success, and you can expect that your coach will listen to you without judgement. Your coach will meet with you at the agreed-upon time and will give you their full attention during each session.

What is NOT Whole Health Coaching?


Whole Health Coaching is not:

Therapy, counseling, and/or clinical care: Your health coach is not trained as a doctor, therapist, or other clinician; instead, your health coach has been trained to motivate, guide, and support you as you work towards your goals. Your health coach cannot diagnose, prescribe medications, evaluate your mental health or medical condition(s) or symptoms, or provide treatment recommendations.

A substitute for professional advice or legal counsel: Your coach will not offer advice, solutions to problems or guidance related to medical, financial, business, spiritual, or other related professional areas. Any decisions made in these areas are assumed to be your responsibility.

How are coaching sessions structured?


The Whole Health Coaching Program includes five (5) sessions with a Whole Health Coach, focused on guided goal setting, planning, and development of a sustained vision that includes your improved overall health and wellbeing. The program begins and concludes with a consultation led by a VEBA Care Navigator – a Critical Care and Holistic Registered Nurse that carefully reviews your personal health history and sets you up for success with a VEBA Whole Health Coach during the program and beyond. The length of the program is between 9 – 11 weeks from start to completion.

What commitments will I be asked to make?


To get the most out of your Whole Health Coaching experience, we ask that you:

Please be on time and ready for the session at the designated start time.

Treat your coach with respect and courtesy.

Be willing to have open and honest communication with your coach.
You have the right to refuse to answer any question you’re uncomfortable with. If you ever feel that your coach doesn’t understand you, it’s important to let them know. While you should still be respectful, you don’t have to adhere to social niceties typically used with friends or family. Be transparent about your thoughts and experiences.

Be fully present for each coaching session, do not multi-task or allow yourself to get distracted. It is recommended that your cell phone or other mobile devices are turned off or silenced during your entire session.

Commit to completing assigned work between sessions. You may be given an exercise or task to work on between coaching sessions based upon the goals you have set, and it is expected that you complete the assignments to the best of your ability, as they are critical to your success in the program.

Session Cancellations, Missed, and/or Rescheduled Appointments

  • If you need to cancel or reschedule a planned Whole Health Coaching session, please contact the VEBA Resource Center by calling (619) 398-4220 within 24 hours of the scheduled date/time of your session.

  • If you do not arrive within 10 minutes of your session start time, you will be considered a “No Show” and you will lose your scheduled session.

  • Missing two or more sessions will significantly impact the member journey and achievement of successful program outcomes. Members should notify the VRC of their desire to discontinue the program should they have difficulty committing to the entire Whole Health Coaching program process and are encouraged to return when they are able to complete the program in its entirety.

Agreement to Participate


I have reviewed the above and understand what to expect from the Whole Health Coaching Program. By agreeing to participate,
    1. I understand that my health and the decisions I make are my responsibility.
    2. I agree to engage fully and appropriately with my coach, as described above, to work towards my wellness goals.
    3. I understand that my coach cannot provide medical advice, diagnosis, or treatment for specific symptoms or medical conditions.
    4. I agree to be available and prepared for my coaching meetings.
    5. I agree to having submitted a Wellness Inventory survey with CA Schools VEBA/the VEBA Resource Center and that the information that I shared is accurate to the best of my knowledge.
By signing this document, you are agreeing to participate in our Whole Health Coaching Program and commit to all agreements as outlined in this document.
